Course Description

Jumping to conclusions without understanding the true origin and source of problems often leads to wasted time and resources. “Root Cause Analysis” is a method of problem solving that involves searching backwards from an undesirable event or outcome to identify its true causes.

Participants will learn how to localize the occurrence of a problem before attempting to identify its root causes, the necessity to agree on the definitions of the problem and how to resolve issues concerning methods of measurement, instrumentations, procedures etc.

Course Content

  • Detrimental problem-solving practices and why organizations often fail at solving problems.
  • Nonconformances and Root Cause Analysis – when it is applied and the generic problem-solving process.
  • Initiating the problem-solving process and defining the problem using basic statistical tools (Pareto Chart), high level flowcharts, and Is / Is Not Analysis.
  • Creating a well-defined Problem Statement.
  • Using flowcharts to understand processes.
  • Identifying probable root causes through the use of 5 Why, Fishbone (Ishikawa) Diagrams, and Interviews.
  • Determining and implementing solutions, including the use of Force Field Analysis.
  • Monitoring processes to ensure that problems do not recur.
  • Incident / Accident Analysis – how incidents and accidents differ from manufacturing or business process issues, and how this affects the Root Cause Analysis process.
  • Process and approach for effective interviewing.
  • Understanding the components and sources of human error.

Prerequisites

This is an introductory course to the concepts of identifying root causes and solving problems.  No previous experience is required.
